Our Policies
Our office hours are subject to change at any time for a variety of reasons, including doctor availability and weather. You can schedule an appointment 24/7. If appointments need to be rescheduled for any reason, the clinic will try to contact you as soon as possible. We recommend calling the clinic to verify your appointment time the day before you are scheduled to come in.
In preparation for your appointment, please be reminded of the following clinic policies:
Patients must bring a valid photo ID to all appointments in order to establish and verify their identity.
Accepted IDs include a driver's license, state ID, or passport.
If you do not have any of these forms of ID, please call the clinic to discuss your options with the clinic staff.
